How do I register as a Patient of the Practice ?

It is easy to register with our practice.

First confirm that you live within our practice area.Our practice area includes Yateley, Eversley, Finchampstead, Little Sandhurst, Sandhurst, Owlsmoor, College Town, Blackwater, Hawley and Minley. Details of our exact boundary can be found at reception.

Then fill in one New Patient Questionnaire for each member of your family.

You can obtain these Questionnaires in a number of ways :

Then, bring the Questionnaires to the Medical Centre TOGETHER WITH your FP1 medical cards (these tell us your NHS number, previous GP etc). If you do not have FP1 medical cards, an alternative form is available at reception.

All patients are allocated a “usual“ doctor and patients are encouraged to see this same doctor each time they come to the surgery. That doctor is responsible for monitoring all correspondence and test results relating to that patient, for updating that patient’s medical records and for managing that patient’s repeat prescriptions. Historically, patients have always preferred to have their “own doctor“ and this system has always worked well. The Oaklands Practice believes that this is the best way of providing good patient care.

However if you wish to choose to see a particular doctor for some or all medical conditions then please let the receptionist know and she will note this in your medical records. Whilst we will always try to ensure that this choice is followed, there may be occasions when it is not possible.
